Protecting your privacy is important

At Physio 360, we are dedicated to safeguarding your personal information. Your personal and medical information will never be shared without your explicit consent.

The personal information we collect includes names, phone numbers, email addresses, physical addresses, and more. Medical information encompasses age, details related to current and past medical conditions, results of previous medical tests, among other things.

The information is provided by the patient verbally or through electronic documents. Electronic documents are managed and stored by GOrendezvous Inc., a clinic management software company. GOrendezvous Inc. is a Quebec-based company, and the data is stored in Canada.

 

Any paper information is kept secure, in a locked filing cabinet and can only be accessed by authorized personnel.

Information sent via email and/or SMS

Any information willingly shared via email or text is used strictly in the person’s best interest and will not be disclosed to third parties without prior consent. When communicating with the clinic through the website’s contact link, you are required to provide a name, telephone number and e-mail address. These emails will be handled with the same level confidentiality outlined in this policy. However, it’s important to note that, despite security measures and password protection, information transmitted over the internet can never be entirely secure.

 

Retention of information

We retain your personal and medical information for a minimum of five (5) years, in accordance to the regulations of the Ordre professionnel de la physiothérapie du Québec. After this period, the information may be securely destroyed or deleted.

 

Access request

Under Article 27 of the Act Respecting the Protection of Personal Information in the Private Sector, you have the right to request access to the personal information collected about you by our clinic. Personal information refers to any information that can directly or indirectly identify you. To do so, please use the contact information below. 

 

Withdrawal of Consent

Consent to collect and maintain your personal information can be withdrawn at any time. This right, however, may be limited by the regulations of the Ordre professionel de la physiothérapie du Québec. For more information please dont hesitate to contact the clinic.
